Parents/guardians are requested to inform the school of any new health problem and/or special health care procedures that need to be performed at school. This will assist the school in developing a plan to best help the student meet their individual needs.
Important information regarding medications:
Medication to be administered to children in school must be accompanied by a Medication Consent Form. These can be faxed to your student’s school office at (715)327-5609.
Non-prescription medications

Require the parent/guardian’s consent/signature.
	 
	- 
	
Only FDA-approved over-the-counter medications will be administered by school personnel.
	 
	- 
	
The school carries a few over-the-counter medications at each school and parents/guardians must consent for their administration by school personnel (i.e. antibacterial ointment, burn gel, Vaseline, Acetaminophen, Ibuprofen, Loratadine).
	 
	- 
	
Over-the-counter medication must be unopened.
	 
	- 
	
Loose pills in a plastic bag, opened bottles, expired medication, or illegible medication bottle labels will not be accepted.
	 
Prescription medications
	- 
	
Require both the parent/guardian’s and Medical Practitioner’s consent/signature. (i.e. Albuterol inhaler, EpiPen, Adderall). 
	 
	- 
	
All medications must be hand-delivered by a parent/guardian to the school in the original container labeled with your child's name, instructions, and Medical Practitioner’s name.
	 
	- 
	
The Medical Practitioner’s order must match the directions on the medication bottle. 
	 
	-

All self-carry Emergency medication (Albuterol, Epinephrine, Glucagon) must have both parent/guardian and Medical Practitioner signature in the state ofWisconsin. 
	 
Hearing Screening: Each year students in kindergarten and first grade are screened for possible hearing problems
Vision Screening: Each year students in grades Kindergarten, one, three, five, seven, and nine re screened for possible vision problems. Parents are notified by phone and letter when a further examination is necessary.
Parents are urged to take care of minor injuries or illnesses which occur at home. Please make sure your child has a healthy breakfast and investigate their complaints if they are feeling ill. 
If your child becomes ill at school, we will contact you. If we cannot reach you, we will use the emergency contact information you have provided. Please remember we cannot keep seriously ill children at school. It is the responsibility of parents to give us current phone numbers for emergency purposes.
